Notice of Privacy Practices

[ Replace this section with your full Notice of Privacy Practices. ]

This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Required under the Health Insurance Portability and Accountability Act (HIPAA), it must be provided to every client and made available publicly.

Your NPP should cover: how protected health information is used for treatment, payment, and operations; the limits of confidentiality including mandatory reporting; your rights to access, amend, and request restrictions on your records; how to file a complaint; and the effective date.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate explaining how much your care will cost.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services. Health care providers are required to give clients who don't have insurance, or who are not using insurance, an estimate of the expected charges for services.

You have the right to receive a Good Faith Estimate in writing at least one business day before your session. You can also ask for one before scheduling. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information, visit www.cms.gov/nosurprises.
